（１）Vaccination

◆Currently Yamato city provides vaccination for different target people as follows.
◆COVID-19 vaccination is not mandatory. Please have a vaccination with the understanding of the effects and the side effects of the vaccines.

Dose	Eligible person
◆First and Second Dose	Age 5＋（a Vaccine suited for children is used for 5 to 11 years old）
◆Third Dose	Age 12＋ and >5 months after the second dose
◆Fourth Dose	>5 months after the 3rd dose and applicable to any of the following; ①Age 60＋ ②18 to 59 yrs old with a particular disease (underlying disease) or an evaluation of doctor as having the high risk of developing severe symptoms.

（２）Voucher

◆A Vaccination voucher issued by your municipality is required for the vaccination. “Your municipality” means the municipality on your certificate of residence (Juminhyo) or the residence card (Zairyu card).
◆If you haven’t received a voucher, or came from a foreign country, we issue a voucher for you. Please contact the Call Center. Your interpreter or representative may help your phone call, or contact on behalf of you

Dose Type	Eligible Person
◆First and Second Dose	A child who will be 5 yrs old will receive a voucher in order (All children over 5 yrs old have already been sent a voucher)
◆Third Dose	All people who completed the second dose four and a half months ago will receive a voucher in order.
◆Fourth Dose	All people who completed the 3rd dose four and a half months ago will receive a voucher in order. Please keep the voucher if ineligible person receives it.

（４）Things to Bring

１）Envelope of vaccination voucher
２）Vaccination voucher
３）Pre-vaccination Questionnaire（Fill in advance with black or blue ballpoint pen）
４）ID（residence card, My Number card, or driver’s license, etc.）
５）Others（prescription log (Okusuri Techo), thermometer, mother and child handbook (Boshi Techo), or reading glasses, etc.）

（５）COVID Vaccine Passport (Vaccination certificate for traveling overseas)

◆It takes about a week to send a passport after receiving an application. The passport is not issued on the same day of application.
※The application of the vaccine passport is expected to increase from June to September. Please apply early.
◆In principle, application of a vaccine passport is only accepted by mail.
◆In case of lack of stamp fee on a return envelope, we will send it by postage paid by addressee.
